That would depend on what file formats SMD can import from makehuman while making sure that you assign a skeleton that supports facial bones both .dae or .fbx support them and are the two main export formats that we have there is also mhx2 which creates a json file that so far has an importer for blender but importers for other 3D modelling apps can be written.

If you are importing to blender then mhx2 is much better gets all of the materials right for both blender internal and cycles as well as all the shape keys for facial bones and visemes and poses as well.

When you exporting from Mh on the export page in the upper right corner see the little check boxes click both expressions and/or poses they need to be checked for the export they show up in blender on the mhx2 runtime tool tab when you are in pose mode only.The old mhx that ships with blender is no longer supported(blender hasn't dropped it yet we have asked for a few releases now) you learn all about mhx2 from https://thomasmakehuman.wordpress.com/ how to install and use.

If you're not afraid to work with not-all-that-friendly and mostly undocumented tools, there's the rigging tools for makehuman too. You could try to make a rig on your own. Not saying it's trivial, but maybe it's worth a shot?

The important thing you need to know is that after you've exported an mhskel file you have to edit it manually to replace the null values with rotation values. Otherwise you'll get a crash in MH when you try to use it.

There is no such tool. You need to open the mhskel file with a text editor and edit it manually.
