Shoe issue

Following grinsegold's tutorial videos on making high heels, I've got a mesh that's ready. But when I go to transfer weights, the pumps go wonky. I don't know what I'm doing wrong.

Not having seen grinsegold's tutorial but just a stab in the dark applying foot pose within MH not blender may do the trick you may need to adjust the pose in blender for the proper height of the heels on your new pumps but the the weighting will already be applied.Then exporting the new pose as a bvh saving it in the v1\data\ special_pose with an appropriate unique name.

This series.

They export using MHX2, then generate the base human in MakeClothes in Blender, then transfer the MHX2 rig to the base female and put the feet into position. and model around that.

Okay. I found a workaround and it all works, except for one thing. When I go to add the pumps to the mannequin in MakeHuman, it throws this error:

Failed to index faces of mesh pumps.obj, you are probably loading a mesh with mixed nb of verts per face (do not mix tris and quads). Or your mesh has too many faces attached to one vertex (the maximum is 5-poles). In the second case, either increase MAX_FACES for this mesh, or improve the mesh topology. Original error message: (<type 'exceptions.ValueError'>) could not broadcast input array from shape (6) into shape (5)

I double checked and there were no tris or ngons. The mesh is all quads.

Did you check for double vertices? When modeling from scratch, it's common to have a few from cancelled extrudes or duplications.
