Body mesh get off when follow path

Dears,

When I assign a character to follow path the body mesh split off and comeback the center point, any idea?!!

the issue was resolved by hit Ctrl+A the apply location+scale+rotation

new issue the when character moving along path it's shrinks up and down, is this related to origin point or that I delete x,y,z root coordinates from bvh file?
