Thanks but that doesn't answer my question. MH has 4 scale units options; decimeter, meter, inch and centimetre. Which one do I use for UE4 and what scale units do I use in Blender so that my model and bones stay the same size throughout the development process?

Well actually it did, the article says to use decimeter- via the photo that they used to illustrate their export process? I figured you would try the tutorial outline provided in that link. Sorry for the confusion?

Based on my research and documentation from Epic, use metric .01 in blender and it will match unreal units when you import. 1 unreal unit = 1 centimeter. You can see 'Send2ue' plugin for Blender, from Epic, for further information and clarification. So, my Blender is set to metric .01. if i export using MHX2 i use centimeters. If i use MPFB i select centimeters. I had issues for quite some time until i did that. Good luck
