Take a look at https://www.youtube.com/watch?v=tAwfR-G813g and https://www.youtube.com/watch?v=eEaVZVbTJOQ

MPFB is included in the zip with 1.2.0, in the addons directory.

I use the Game Engine skeleton under Pose/Animate then export as FBX. Then in Blender, select the skeleton, go into Pose mode Ctrl+A, and choose Apply Pose as Rest Pose. The mesh will fall flat on it's face, but then you go into object mode and select the mesh and set X rotation to -90. Then ctrl+A and set scale and rotation of the mesh. There might be a better way, but this is how I do it.

Yes, we should probably fix this in the exporter given that Blender is by far the main destination of Makehuman figures. Blender uses a z-up universe (like I think most CAD programs and 3DS Max do), but Makehuman uses a y-up universe like Maya. We could put it down as a feature request in case the FBX exporter gets some love. In the meantime, I think the 2.79 Blender importer used to let you specify orientation. Haven't looked for a long time.

You don't need to save the model at all before importing (with MPFB). The idea is that blender talks directly with the makehuman program and fetches the data without the need to first save/export.
