Merging Make Human model with custom modeled head/face model

I'd like to create a model of myself with a Make Human body and a custom modeled head (face). I want to create facial mocap (I know how to do this in theory) and also want the whole model to be walking, dancing etc. Is it possible to merge a MH body with a custom head model? If yes, then how? Can I easily adjust the rig then? I tried looking this up on youtube but didn't find anything useful.
