I placed a MakeHuman character over an 3D scan of a real body and baked. Not because it was easy, but because it was hard
. At the beginning I thought of sculpting but felt I havent had the skills. To paint a height map/displacement map was nether an option as it would also need some skills to get it right, as all bodys have tiny unevenness that would be too time consuming to paint, at that time, but now, I would consider making an displacement from an BW photo at least when adding some larger details, like Jennifer's knees is baked from an displacement texture made from a real knee photo. I recommend doing all baking in Blender Internal as it is much faster and easier to manage.
